Submission Deadline

All documents are due 45 days after the termination date of your project.

Submission Requirements

Except as noted below, reports must be provided in hard copy and electronic formats.

_____ Hard Copies: Please submit six (6) hard copies of the DOT Final Report. Submit one (1) hard copy of the UTCM leverage documentation. Submit all documents to Martha Raney Taylor in the UTCM Business Office.

_____ Electronic Copy: Please submit your Final Report in one complete file in PDF format (preferred) or MS Word via e-mail to Martha Raney Taylor. EXCLUDE the UTCM documentation of leveraging from your electronic submission. Please be careful with images that are imported into the document, including image file size and placement within the document.  This is especially critical if you submit a Word document.  If you have concerns with file sizes or file formats, please contact Martha Raney Taylor.

UTCM Documentation of Leverage

(FY07 and FY08 Projects only)

For projects awarded in FY07 and FY08 (RF project numbers ending in -00002 through -00027), if you had leverage from university or other sources, you provided a letter of commitment from each source to provide that leverage. You must now document that this leverage was indeed provided. For each leverage source, complete the Documentation of Leverage form (see box, top right) and attach a copy of the previously submitted document committing the leverage. (If you need copies of your commitment letters, please contact Martha Raney Taylor.) Submit ONE hard copy to the UTCM. This UTCM requirement is separate from the DOT Final Report.
